Submit a Claim

  1. Acquire a copy of the member's ID card.
  2. Send the original, fully itemized bill to the claims address shown on the member's ID card.
  3. If you have questions, please call Allied Client Services to verify benefits.

Dental ID Card

On the member's dental ID card, the claims address
is located on the back of the card, not the front.

Reference-Based Pricing

Many of our Allied members have a plan that allows them to see any provider they choose without restrictions or penalties. There are no preferred providers or networks required. Reimbursement to providers is reference based on a multiple of Medicare reimbursement levels.
